“I must say that the entire staff works at a very friendly and professional level.   They make the experience as pleasant as possible, regardless of the issue.  Obviously, my favorite is Jenn, but wouldn’t second guess going to see anyone in the office.”    Michelle Oberhofer

Jenn was born and raised in Cleveland, Ohio and moved to Missoula, in 2001.  She graduated with her Physician Assistant degree from the MEDEX Northwest Program in 2012.  Jenn has been a PA for ten years and has extensive primary care experience. She is proficient in numerous procedures including Nexplanon placements, toenail removals, skin biopsies and procedures in the urgent care setting. Prior to CostCare, she worked at Providence Medical Group – Missoula Family Medicine where she helped to build the practice from one provider to five. Jenn enjoys working in family medicine and urgent care and is excited to help you on your medical journey.  Jenn lives in Missoula with her two dogs and keeps busy running, hunting, fishing and skiing.  She enjoys being very active and loves the opportunity living in Missoula offers.

Sarianna Rembe

Provider, PA-C

Sarianna (Sari) was born and raised in Missoula. She attended Montana State University for her undergraduate degree and the University of Washington for her PA training, graduating with honors. She is very grateful to be able to practice in her hometown. Sari believes her role as a provider is to be a partner and advocate for her patients. She loves hiking, spending time on the water, traveling, and going on adventures with her husband, Tevan.

Lindsay Kindelman-Lande

Provider, FNP-BC

Born in Billings and shaped by Montana’s wide-open spaces, Lindsay moved to Missoula to attend the University of Montana and never looked back. With more than two decades of nursing experience, she has cared for Montanans across every level of need—from her early days as a certified nursing aide, to serving as a licensed practical nurse, and later as a registered nurse at St. Patrick Hospital. She earned her Master of Science in Nursing from Gonzaga University in 2013 and has since brought her expertise to physical medicine and rehabilitation, long-term care, urgent care, and primary care. At CostCare, that depth of experience translates into what matters most to our patients: someone who listens, someone who partners, and someone who believes preventive care is the heart of staying well. Lindsay meets people where they are and helps them build health goals that actually fit their lives—one practical step at a time. When she’s not in the clinic, you’ll find her with her family—her husband, her dogs, her cat, and her charismatic flock of chickens. She’s also a passionate musician (especially jazz) and feels most at home outdoors, whether she’s running, biking, or hiking with her dogs along Missoula’s trails.

Conor Dysinger

PROVIDER, FNP-C

Conor received his bachelor’s in nursing from Montana State University and his Master’s in Nursing from University of Colorado. Prior to joining CostCare, Conor worked as an RN in inpatient medicine, medical oncology and surgical services for ten years in the Missoula area. He decided to pursue advanced practice for his interest in preventative medicine, providing early intervention, and increasing access to healthcare. Conor values spending time with patients discussing lifestyle changes along with providing relevant and up to date medicine. Conor enjoys spending time in the mountains, skiing, and mountain biking.

“I started using CostCare as my provider because they are preferred for my insurance and employer.  I was apprehensive at first as I didn’t want a doctor that just shoved their patients through.  My first experience was with Jenn Culp and I was pleasantly surprised.  She is very knowledgeable, thorough and has a great sense of humor.  My entire family are now patients as well. I have 2 teenage girls and one of them can be a bit overdramatic at times. Regardless of the situation, Jenn and all the other staff take it in stride and put a smile on her face before she leaves the office.  My husband, who avoids doctors like the plague, is now comfortable enough to call for an appointment himself.” Michelle Oberhofer

It's a great time to get healthy!
